Блокатори β-адренорецепторів у лікуванні серцевосудиних захворювань: сучасні позиції карведилолу

Анотація

Стаття присвячена ролі блокаторів β-адренорецепторів (БАБ) у лікуванні серцево-судинних захворювань. На основі результатів численних великих клінічних досліджень автор демонструє ефективність даної групи препаратів, що можуть впливати не тільки на симпатичну активність, зумовлюючи зниження симптомів гіперсимпатикотонії, а й на прогноз пацієнтів із серцево-судинними захворюваннями. Позитивні клінічні ефекти даної групи препаратів показані на прикладі представника групи БАБ карведилолу: препарат чинить β- і α-адреноблокуючий ефект і має антиоксидантну, протизапальну, антиапоптичну активність, є метаболічно нейтральним, що дозволяє його призначати пацієнтам з артеріальною гіпертензією і цукровим діабетом, а також пацієнтам з метаболічним синдромом, що особливо важливо при лікуванні осіб похилого віку. Препарати групи БАБ виявляють позитивну дію на фактори ризику серцево-судинних захворювань і забезпечують захист органів-мішеней у хворих на артеріальну гіпертензію, пацієнтів з хронічною серцевою недостатністю, фібриляцією передсердь і після перенесеного інфаркту міокарда. Вибір на користь перевірених БАБ, знання показань і протипоказань до їх призначення, можливість попереджати їх негативні ефекти розширюють лікарські можливості застосування БАБ у клінічній практиці
