До питання гепатотоксичності інгібітора тирозинкінази іматинібу у пацієнтів з хронічною мієлоїдною лейкемією

Стаття присвячена гепатоцелюлярним ускладненням терапії хронічної мієлоїдної лейкемії. На сьогодні досягнуті певні успіхи в лікуванні пацієнтів з онкогематологічними захворюваннями, зокрема хронічною мієлоїдною лейкемією, що, насамперед, пов’язано з розвитком таргетної терапії, а саме – зі створенням специфічних інгібіторів тирозинкінази (іматинібу, нілотинібу, дазатінібу та ін.). Поява іматинібу кардинально змінила підходи до терапії хронічної мієлоїдної лейкемії, а поява слідом за ним препаратів другої лінії (нілотинібу, дазатинібу) збільшила ймовірність розвитку ремісії у випадках резистентності до терапії іматинібом. Однак поряд з високою ефективністю ці препарати мають певну токсичність. У статті детально розглянуто ураження печінки на фоні терапії інгібітором тирозинкінази – іматинібом. Наводяться розширені дані світового досвіду застосування даного препарату з позиції його гепатотоксичності. Обговорюється моніторинг функції печінки на фоні терапії іматинібом і варіанти корекції іматиніб-індукованої гепатотоксичності.
