Сучасні досягнення і перспективи застосування небівололу в лікуванні пацієнтів із кардіоваскулярними захворюваннями

Огляд присвячений клінічному застосуванню бета-адреноблокатора (БАБ) з вазодилатаційними властивостями – небівололу. Розглянуто основні фармакологічні властивості та характеристики препарату, його кардіоселективність та механізми вазодилатаційної дії. Показано особливе місце небівололу в лікуванні есенціальної артеріальної гіпертензії (АГ), ішемічної хвороби серця (ІХС) та серцевої недостатності (СН). Препарат не чинить діабетогенної дії при тривалому застосуванні, сприятливо впливає на показники ліпідного та вуглеводного обмінів. У хворих на ІХС демонструє антиангінальну та антиішемічну дії, знижує частоту рецидивів після виконання операцій на коронарних артеріях (КА). Препарат добре переноситься та ефективно знижує показники смертності у пацієнтів з хронічною СН, старших 70 років, незалежно від фракції викиду лівого шлуночка. У лікуванні ІХС при цукровому діабеті 2-го типу перевагу надають вазодилатаційним бета-адреноблокаторам. При необструктивних ураженнях КА небіволол обирають серед існуючих БАБ для лікування мікросудинної стенокардії. Застосування небівололу в пацієнтів з АГ характеризується, крім антигіпертензивного ефекту, сприятливим впливом на функцію ендотелію, метаболічний профіль, центральну гемодинаміку, якість життя пацієнтів, а також сприятливішим профілем побічних ефектів.
